To notify us of changes (scope, address, number of site(s), etc.) during your certification cycle, please fill in this form and send it to fffs_apply@fami-qs.org, specifying in your email all the changes to be made.

Please note that an administrative fee of 150.00€ will be applied to process the changes. In case of registration of new site(s), the annual service fee is applied (per site) and, therefore, no administrative fee will be charged.
